I've been so obsessed over this that I've downloaded full clean saves of the game after reinstalling it with no mods and literally only use F4SE and the required mods for the preset. Including looksmenu compendium and looksmenu standalone. In my case, I fiixed it by activating the Looksmenu bug fixes in the Buffout 4 configuration file. I think they call it the exploding heads problem. Not too long ago I updated and installed a few mods like, F4SE, Looks Menu Customization Compendium (LMCC), LooksMenu its self, and I installed a mod called Real HD Face Textures 2k (also using the LMCC patch for it), in addition to that I also installed the Pip-Pad mod and the proper LMCC patch that goes along with that.
